Noi, il sottoscritto consorzio di scienziati internazionali, siamo profondamente preoccupati per la costante e inaccurata presentazione fornita dai mass-media sul Disturbo da Deficit d'Attenzione ed Iperattività (ADHD).

Esso rappresenta un disturbo che tutti noi ben conosciamo e per il quale molti di noi hanno dedicato studi se non addirittura intere carriere professionali. Temiamo che resoconti imprecisi che presentino l'ADHD come mito, frode o condizione benigna possano indurre migliaia di pazienti a non cercare la terapia per il loro disturbo. Ciò lascia anche l'opinione pubblica con la sensazione che il disturbo non fondato o reale o rappresenti un problema piuttosto banale.

Abbiamo predisposto questo accordo sull'ADHD come riferimento sullo stato delle scoperte scientifiche che riguardano questo disturbo, la sua validità e il suo grave impatto sulla vita dei soggetti diagnosticati di cui al presente documento.

Chi presenta occasionalmente il disturbo tratta il problema alla stregua di un evento sportivo con avversari che regolarmente s'incontrano. Le opinioni di un gruppetto di dottori non esperti che affermano che l'ADHD non esiste sono poste a confronto con le consolidate opinioni scientifiche che affermano il contrario, come se entrambe le opinioni potessero godere eguali meriti. Tali tentativi alla fine danno all'opinione pubblica la sensazione che vi sia un sostanziale disaccordo scientifico sul fatto che l'ADHD sia un disturbo reale. Infatti, non esiste affatto tale disaccordo almeno non più di quanto ve ne sia sul concetto che, ad esempio, il fumo possa causare il cancro o che un virus l'HIV causi l'AIDS.

L'Associazione americana di Chirurgia, l'Associazione Medica Americana (AMA), l'Associazione Psichiatrica Americana, l'Accademia Americana di Psichiatria dell'Infanzia e dell'Adolescenza (AACAP), l'Associazione Americana di Psicologica e l'Accademia Americana di Pediatria (AAP), tra le altre, riconoscono tutte l'ADHD come fondato disturbo. Mentre alcune di tali Comunità scientifiche hanno emanato linee guida per la diagnosi e la terapia del disturbo, questo è il primo accordo emanato da un consorzio indipendente di scienziati eminenti riguardante lo "stato dell'arte" sul disturbo.

Tra gli scienziati che hanno dedicato anni, se non intere carriere professionali allo studio di questo disturbo non v'è alcuna controversia riguardo la sua esistenza.
 

L'ADHD e la Scienza

Noi non potremo mai sottolineare abbastanza il fatto che, come dato scientifico, l'affermazione che l'ADHD non esista è semplicemente errata. Tutte le principali associazioni mediche e le agenzie governative sulla salute riconoscono l'ADHD come un disturbo effettivo a causa dell'evidenza scientifica che stabilisce la cosa in modo inconfutabile.

Vari approcci sono stati usati per stabilire se una certa condizione possa assurgere al livello di un fondato, reale disturbo medico o psichiatrico. Un approccio molto utile in tal senso stabilisce che vi deve essere una evidenza scientifica che i pazienti che soffrano di una data condizione manifestino una seria mancanza o uno scadimento di un meccanismo fisico o psicologico che sia comune a tutti gli uomini. Cioè, ci si aspetta che tutti gli uomini, normalmente e indipendentemente dalla cultura, abbiano sviluppato quella data capacità mentale. E vi deve essere una evidenza scientifica, egualmente incontrovertibile, che questa seria mancanza arrechi un pregiudizio al soggetto. Il pregiudizio è valutato sulla base dell'aumentata mortalità, degli aumentati stati di morbosità o scadimento nelle principali attività della vita richiesta in un determinato stadio del suo sviluppo. Le attività più importanti della vita sono quegli ambiti di funzionamento come l'educazione, le relazioni sociali, le interazioni familiari, l'indipendenza e l'autosufficienza, la funzionalità a livello lavorativo e che ci si aspetta che tutti gli uomini raggiungano in determinati momenti dello sviluppo.

Come confermato dai numerosi scienziati che sottoscrivono questo documento, non sussiste alcuna controversia tra gli eminenti ricercatori a livello clinico che l'ADHD attivi una grave mancanza in un insieme di capacità psicologiche e che queste mancanze creino grave pregiudizio alla maggior parte degli individui che manifestano il disturbo. L'attuale evidenza scientifica indica che i deficit nella inibizione comportamentale e nell'attenzione sostenuta siano i fattori centrali del disturbo, fatti dimostrati attraverso centinaia di studi scientifici. E non v'è dubbio alcuno che l'ADHD comporti uno scadimento nelle attività principali della vita, comprese le relazioni sociali, l'educazione, le relazioni familiari, la funzionalità lavorativa, l'autosufficienza e il rispetto delle regole sociali, delle norme e delle leggi. L'evidenza mostra anche che i soggetti con ADHD hanno maggiore predisposizione a procurarsi danni fisici e intossicazioni accidentali. Questo è il motivo per cui nessuna organizzazione medica professionale, psicologica o scientifica dubita dell'esistenza dell'ADHD come disturbo reale.

I deficit centrali di tipo psicologico nei soggetti con ADHD sono stati collegati attraverso numerosi studi, con l'utilizzo di svariate metodiche scientifiche, a numerose regioni specifiche del cervello (il lobo frontale, i suoi collegamenti ai gangli della base e la loro relazione alle posizioni centrali del cervelletto). La maggior parte degli studi neurologici hanno determinato che i soggetti con ADHD come gruppo hanno minor attività elettrica e mostrano minore reattività alla stimolazione in una o più di queste regioni cerebrali. Inoltre, studi di neuro-immagine di gruppi di soggetti con ADHD mostrano aree più piccole di materia cerebrale e di minore attività metabolica rispetto a quanto accade nei gruppi di controllo utilizzati in tali studi.

Si è scoperto in numerosi studi di gemelli omozigoti ed eterozigoti condotti in varie nazioni (Stati Uniti, Gran Bretagna, Norvegia, Australia, etc.) che gli stessi deficit psicologici nell'inibizione ed attenzione sono stati primariamente ereditati. Si è riscontrato che il contributo genetico in tali caratteri è di norma tra i più elevati di qualunque disturbo psichiatrico (dal 70 al 95% della variazione del carattere nella popolazione), risultando all'incirca simile al contributo genetico relativo all'altezza umana. E' stato dimostrato in modo attendibile che un particolare gene è associato a questo disturbo ed inoltre, in questo momento, più di dodici differenti team scientifici stanno conducendo indagini su altri geni.

Numerosi studi sui gemelli dimostrano che l'ambiente familiare non fornisce un significativo contributo specifico a questi aspetti. Ciò non significa che l'ambiente familiare, le capacità di gestione dei genitori, eventi particolarmente stressanti della vita, relazioni con coetanei a rischio non siano importanti o non abbiano qualche influenza sui soggetti con tale disturbo. Le tendenze genetiche sono espresse nell'interazione con l'ambiente. Inoltre, i soggetti con ADHD spesso hanno associati altri disturbi e problemi alcuni dei quali sono chiaramente collegati ai loro ambienti sociali. Ma v'è da dire che i deficit psicologici sottostanti che costituiscono l'ADHD non sono da soli o in modo primario il risultato di questi fattori ambientali.

E' questo il motivo per cui i più importanti scienziati mondiali, quali i sottoscritti, riconoscono l'evidenza sempre maggiore dei contributi neurologici e genetici a questo disturbo. Tale evidenza associata con innumerevoli studi sul danno recato dal disturbo e centinaia di studi sull'efficacia del farmaco, comporta la necessità in molti casi, sebbene non in tutti, di un approccio al disturbo con terapie multiple. Queste prevedono il farmaco combinato con interventi anche a livello educativo, familiare e sociale. Ciò è in straordinario contrasto con tutte le prospettive non scientifiche di alcune critiche di natura sociale che appaiono sui media e che ritengono l'ADHD un imbroglio, che sottoporre a trattamento farmacologico i soggetti che ne sono afflitti sia opinabile se non addirittura da riprovare e che qualunque problema associato all'ADHD sia esclusivamente il risultato di problemi familiari, eccessivo uso della TV o di videogiochi, di diete, mancanza di affetto ed attenzione, intolleranza da parte della scuola e degli insegnanti.

L'ADHD non è un disturbo benigno. Per coloro che ne sono afflitti l'ADHD può causare problemi devastanti. Studi di controllo (follow-up) su campioni clinici riportano che i pazienti ADHD tendono più della norma all'abbandono scolastico (32-40%), a completare raramente gli studi (5-10%), ad avere pochi amici o addirittura nessuno (50-70%), ad avere uno scarso rendimento nel lavoro (70-80%), ad attivare comportamenti antisociali (40-50%) ed usare tabacco o droghe vietate più degli altri coetanei. Inoltre, i ragazzi che crescono con l'ADHD tendono ad avere esperienze di gravidanza precoce (40%) e malattie trasmesse sessualmente (16%), a parlare eccessivamente e ad avere molti incidenti automobilistici, a sperimentare la depressione (20-30%) e disturbi della personalità da adulti (18-25%) e in centinaia di altri modi gestire male e rovinare la loro vita.

Tuttora, nonostante queste gravi conseguenze, gli studi indicano che meno della metà dei soggetti con il disturbo stanno ricevendo un trattamento. I media possono aiutare considerevolmente a migliorare tale situazione. Possono fare in modo di descrivere l'ADHD e le evidenze scientifiche che lo sottendono quanto più accuratamente e responsabilmente possibile, non sostenendo la propaganda delle critiche sociali e di dottori estremisti le cui azioni politiche vorrebbero che l'opinione pubblica ritenesse che non si tratta di un vero disturbo. Pubblicare storie in cui l'ADHD viene presentato come un disturbo fittizio o semplicemente un conflitto fra gli attuali gianburrasca e i loro educatori equivale ad affermare che la terra è piatta, le leggi della gravità sono discutibili e la tavola periodica degli elementi della chimica è un imbroglio.

L'ADHD dovrebbe essere descritto dai media altrettanto realisticamente ed accuratamente quanto è descritto dalla scienza, come un reale disturbo che ha un sostanziale e variegato impatto negativo nei confronti di coloro che ne soffrono non per colpa dei loro genitori e degli insegnanti.

We, the undersigned consortium of international scientists, are deeply concerned about the periodic inaccurate portrayal of attention deficit hyperactivity disorder (ADHD) in media reports. This is a disorder with which we are all very familiar and toward which many of us have dedicated scientific studies if not entire careers. We fear that inaccurate stories rendering ADHD as myth, fraud, or benign condition may cause thousands of sufferers not to seek treatment for their disorder.
It also leaves the public with a general sense that this disorder is not valid or real or consists of a rather trivial affliction.
We have created this consensus statement on ADHD as a reference on the status of the scientific fíndings conceming this disorder, its validity, and its adverse impact on the lives of those diagnosed with the disorder as of this writing (January 2002).
Occasional coverage of the disorder casts the story in the form of a sporting event with evenly matched competitors. The views of a handful of non-expert doctors that ADHD does not exist are contrasted against mainstream scientific views that it does, as if both views had equal merit. Such attempts at balance give the public the impression that there is substantial scientific disagreement over whether ADHD is a real medical condition. In fact, there is no such disagreement --at least no more so than there is over whether smoking causes cancer, for example, or whether a virus causes HIV/AIDS.
The U.S. Surgeon General, the American Medical Association (AMA), the American Psychiatric Association, the American Academy of Child and Adolescent Psychiatry (AACAP), the American Psychological Association, and the American Academy of Pediatrics (AAP), among others, all recognize ADHD as a valid disorder. While some of these organizations have issued guidelines for evaluation and management of the disorder for their membership, this is the first consensus statement issued by an independent consortium of leading scientists conceming the status of the disorder.
Among scientists who have devoted years, if not entire careers, to the study of this disorder there is no controversy regarding its existence.  

 ADHD and Science
 
We cannot over emphasize the point that, as a matter of science, the notion that ADHD does not exist is simply wrong. All of the major medical associations and goverment health agencies recognize ADHD as a genuine disorder because the scientifíc evidence indicating it is so is overwhelming.
Various approaches have been used to establish whether a condition rises to the level of a valid medical or psychiatric disorder. A very useful one stipulates that there must be scientifically established evidence that those suffering the condition have a serious deficiency in or failure of a physical or psychological mechanism that is universal to humans. That is, all humans normally would be expected, regardless of culture, to have developed that mental ability.
And there must be equally incontrovertible scientific evidence that this serious deficiency leads to harm to the individual. Harm is established through evidence of increased mortality, morbidity, or impairment in the major life activities required of one's developmental stage in life. Major life activities are those domains of functioning such as education, social relationships, family functioning, independence and self-suffíciency, and occupational functioning that all humans of that developmental level are expeeted to perform.
As attested to by the numerous scientists signing this document, there is no question among the world’s leading clinical researchers that ADHD involves a serious deficiency in a set of psicological abilities and that these deficiencies pose serious harm to most individuals possessing the disorder. Current evidence indicates that deficits in behavioral inhibition and sustained attention are central to this disorder -- facts demonstrated through hundreds of scientific studies. And there is no doubt that ADHD leads to impainnents in major life activities, inclina social relations, education, family functioning, occupational functioning, self-sufficiency, and adherence to social rules, norms, and laws. Evidence also indicates that those with ADHD are more prone to physical injury and accidental poisonings. This is why no professioni medical, psicological or scientific organization doubts the existence of ADHD as a legitimate disorder.
The central psychological deficits in those with ADHD have now been linked through numerous studies using various scientific methods to several specific brain regions (the frontal lobe, its connections to the basal ganglia, and their relationship to the central aspects of the cerebellum). Most neurological studies find that as a group those with ADHD have less brain electrical activity and show less reactivity to stimulation in one or more of these regions. And neuro-imaging studies of groups of those with ADHD also demonstrate relatively smaller areas of brain matter and less metabolic activity of this brain matter than is the case in control groups used in these studies.
These same psychological deficits in inhibition and attention have been found in numerous studies of identical and fraternal twins conducted across various countries (US, Great Britain, Norway, Australia, etc.) to be primarily inherited. The genetic contribution to these traits is routinely found to be among the highest for any psychiatric disorder (70- 95% of trait variation in the population), nearly approaching the genetic contribution to human height. One gene has recently been reliably demonstrated to be associated with this disorder and the search for more is underway by more than 12 different scientifíc teams worldwide at this time.
Numerous studies of twins demonstrate that family environment makes no significant separate contribution to these traits. This is not to say that the home environment, parental management abilities, stressful life events, or deviant peer relationships are unimportant or have no influence on individuals having this disorder, as they certainly do. Genetic tendencies are expressed in interaction with the environinent. Also, those having ADHD often have other associated disorders and problems, some of  which are clearly related to their social environments. But it is to say that the underlying psychological deficits that comprise ADHD itself are not solely or primarily the result of these environmental factors.
This is why leading intemational scientists, such as the signers below, recognize the mounting evidence of neurological and genetic contributions to this disorder. This evidence, coupled with countless studies on the harm posed by the disorder and hundreds of studies on the effectiveness of medication, buttresses the need in many, though by no means all, cases for management of the disorder with multiple therapies. These include medication combined with educational, family, and other social accommodations. This is in striking contrast to the wholly unscientific views of some social critics in periodic media accounts that ADHD constitutes a fraud, that medicating those afflicted is questionable if not reprehensible, and that any behavior problems associated with ADHD are merely the result of problems in the home, eccessive viewing of TV or playing of video games, diet, lack of love and attention, or teacher/school intolerance.
ADHD is not a benign disorder. For those it afflicts, ADHD can cause devastating problems. Follow-up studies of clinical samples suggest that sufferers are far more likely than normal people to drop out of school (32-40%), to rarely complete college (5-10%), to have few or no friends (50-70%), to under perfonn at work (70-80%), to engage in antisocial activities (40-50%), and to use tobacco or illicit drugs more than normal. Moreover, children growing up with ADHD are more likely to experience teen pregnancy (40%) and sexually transmitted diseases (1 6%), to speed excessively and have multiple car accidents, to experience depression (20-30%) and personality disorders (18- 25%) as adults, and in hundreds of other ways mismanage and endanger their lives.
Yet despite these serious consequences, studies indicate that less than half of those with the disorder are receiving treatment. The media can help substantially to improve these circumstances. It can do so by portraying ADHD and the science about it as accurately and responsibly as possible while not purveying the propaganda of some social critics and fringe doctors whose political agenda would have you and the public believe there is no real disorder here. To publish stories that ADHD is a fictitious disorder or merely a conflict between today's Huckleberry Finns and their caregivers is tantamount to declaring the earth flat, the laws of gravity debatable, and the periodic table in chemistry a fraud. ADHD should be depicted in the media as realistically and accurately as it is depicted in science -- as a valid disorder having varied and substantial adverse impact on those who may suffer from it through no fault of their own or their parents and teachers.
